ResponsibilitiesPerforms patient assessments andcollaborates with the care team to develop and implement a plan ofcare.
Makes referrals to other disciplines as indicated by thepatient’s identified needs or documents rationale for not doingso.
Promotes patient health and independence through teaching andappropriate rehabilitative measures, assisting patients in learningappropriate self-care techniques.
Supervises LPNs and HHAs.
Completesdocumentation timely and accurately.
Regularly communicate patientprogress to the clinical manager and care team.
Plans and providesstaff education.
Performs on-call responsibilities and on-callservices to patients/families as assigned.
Participates in clinicaldevelopment and continuing education programs.
Other duties asassigned.
QualificationsOne (1+) year of clinical experience as aRegistered Nurse (RN). If less than 1 year clinical experience as aRN, candidate must be approved by VP Clinical.
- Current RN license,specific to the state(s) you are assigned to work.
Current CPRcertification.
Valid driver’s license, reliable transportation andliability insurance.
