The rolerequires effective communication with interdisciplinary teams andparticipation in patient discharge processes to ensure the highestquality of care. This position necessitates an Associate’s degreein nursing, along with appropriate licensure and certifications.

• Provide planning and delivery of direct and indirect patientcare through the nursing process of Assessment, Planning, Intervention, and Evaluation.
• Develop nursing care plans in coordination with patients,family, and interdisciplinary staff as necessary.
• Communicate changes in patient’s clinical condition withPhysicians, Nursing Supervisor/Manager, and co-workers as appropriate.
• Participate in discharge planning process.

Qualifications Minimum Education Associate’s degree required

Certifications:

Basic Life Support (BLS

• -must be BLS certified within 30 days fromhire date.

Required Skills License to practice as a Registered Nurse in the
