Position Summary:
Coordinates efforts to obtain and maintain certifications ofexcellence including Heart Failure and Stroke. Coordinates QIefforts, either within a department or withing a larger QA team,related to data collection and reporting for Disease SpecificCertifications/Accreditations.
Duties and Responsibilities:
Concurrently coordinates all activities / metrics / standardcompliance related to disease specific care for HF, Stroke Collects, enters, analyzes, and presents QI data andactivities. This includes Core measure data; hospital and medicalstaff quality data, required measure data, complaint and occurrencedata and peer review data as related to Disease Specific care. Facilitates hospital-wide compliance with certificationspecific accreditation standards. Champions performance improvement teams for disease specificcare Provides staff education and serves and a disease specificresource for hospital leaders and staff Lead accreditation survey activities Provides Quality orientation for new employees Participates in interdisciplinary team rounds Demonstrates problem-solving skills and implements processes toimprove patient care delivery Lead community outreach efforts related to disease specificcare
Qualifications:
Required Education:
Bachelor’s degree is required
Required License:
Registered Nurse in North Carolina is required (or compactstate)
Required Certification:
Heart Failure Nurse Certification preferred
Required Experience:
Minimum 2-3 years clinical hospital experience
