Develop, implement, and review individualized care plans in collaboration with an interdisciplinary team. Ensure compliance with federal and state regulations for MDS assessments and care planning. Monitor and transmit MDS assessments to the relevant authorities. Train and guide nursing staff on MDS procedures and updates. Audit clinical records to ensure accuracy, completeness, and regulatory compliance. Collaborate with healthcare providers to optimize resident care outcomes. Qualifications Active RN license required. MDS Certification preferred. Proven experience in clinical assessment and MDS coordination. Strong knowledge of federal and state regulations related to MDS and care planning. Exceptional organizational skills and attention to detail.
