Job Summaryand QualificationsProvides coordinated skilled nursing care topatients of all age groups, in the home. Demonstratesaccountability and responsibility in collaborating with theinterdisciplinary team to establish and achieve patient goals andmaintain high quality patient care. Performs in accordance withphysicians orders and under the supervision of the ClinicalManager. What you will do in this role: Assesses home care patients identifying physical, psychosocialand environmental needs as evidenced by documentation, clinicalrecords, case conferences, team reports, call-in logs and on-siteevaluations. Completes OASIS, assessment and visit paperwork according toagency policy. Assures clinical notes accurately indicatecontinuing communication and coordination of services with thephysician, other interdisciplinary team members andpatient/family/caregiver. Communicates significant findings, problems and changes toClinical Manager and physician, and documents all findings,communications, and appropriate interventions. Supervises and provides clinical direction to home health aidesand LPNs/LVNs to ensure quality and continuity of servicesprovided. Responsible for participating in on-call rotation and emergencycall according to agency policy.

What qualifications you will need:

Basic Cardiac Life Support must be obtained within 30 days ofemployment start date Registered Nurse Drivers License Associate Degree, or Registered Nurse DiplomaHealthcare at Homeis a service line from HCA healthcare.
