The ideal candidate will play a crucial role in ensuring compliance with regulatory requirements while providing high-quality patient care. This position involves coordinating assessments, managing documentation, and collaborating with interdisciplinary teams to enhance patient outcomes. Duties Conduct comprehensive assessments of patients’ needs and develop individualized care plans. Ensure compliance with MDS (Minimum Data Set) standards and regulations. Collaborate with healthcare professionals to facilitate discharge planning and utilization management. Perform utilization reviews to assess the appropriateness of care provided in various settings, including ICU and OR. Maintain accurate documentation in eClinicalWorks and other electronic health record systems. Monitor HEDIS (Healthcare Effectiveness Data and Information Set) measures to improve patient care quality. Provide education and support to staff regarding MDS processes and standards. Participate in quality assurance initiatives to enhance overall patient care.
Responsibilities:
Complete and submit MDS assessments in a timely and accurate manner Collaborate with interdisciplinary team members to develop, implement, and update resident care plans Monitor changes in resident condition and update assessments as needed Ensure compliance with all CMS regulations related to MDS and RAI process Participate in Quality Assurance and Performance Improvement (QAPI) initiatives Educate and support nursing staff in documentation practices to ensure accuracy and consistency
Qualifications:
Valid RN license Prior MDS experience preferred Knowledge of federal and state long-term care regulations Proficiency in electronic medical records and MDS software ( PointClickCare) Strong organizational skills and attention to detail Excellent communication and teamwork abilities Join our team as an MDS Coordinator and contribute to delivering exceptional healthcare services while ensuring compliance with industry standards.
