JOB DESCRIPTION AND QUALIFICATIONS

To provide the fullest possible educational opportunity for each student by minimizing absence due to illness and creating a climate of health and well-being in the schools.
Performance and Responsibilities:
Work under the supervision of the school principal, the Special Services Coordinator, the Lead Nurse or their designee. Maintain the health room. Keep an inventory of all equipment and supplies. Project a favorable image of the school and school system in working with students, parents, and community. Render services in support of the school program. Perform assigned duties accurately, promptly, courteously, and efficiently. Accept instructions and strive continuously to improve competencies needed to perform assigned duties. Assume responsibility for selection and referral of students in need of medical and dental care. Assume authority, in the absence of a physician, for the care of a student or staff member who has suffered injury or emergency illness. Visit students’ homes when necessary. Contact homes of children referred by principals. Authorize exclusion and readmission of students in connection with infectious and contagious disease. Advise teachers on health matters, particularly regarding screening for student health defects. Attend committee meetings, workshops, and conferences regarding health service and health curriculum. Participate in in-service training programs. Provide a state approved course to non-licensed assistive personnel and periodic evaluations and monitoring. Conduct pediculosis (head lice) screening in accordance with School Board policy and procedures and re-evaluate students for readmittance to class as necessary. Conduct vision screening as directed. Conduct hearing screening as directed. Conduct scoliosis screening for grades five through nine. Develop and implement Individual Health Care Plans for all students on medications, special health care needs, and chronic illness. Review all immunization records or Blue Cards for compliance. Provides notification to parents upon expiration. Document health room visits in a confidential manner. Maintains individual log; no “block” charting is acceptable. Document medications on the form(s) provided by the State Board of Nursing. Ensure completion of and filing of Prescription Authorization Forms in the student health room file. Maintain records of students with inhalers for asthma; parent and prescriber forms must be on file for students to keep their inhalers with them. Complete end of the school year State Board of Education forms documented monthly and filled out completely in May; to be mailed to the State Board of Education the first of June. Provide annual in-service for faculty and staff on Communicable Disease/Blood Borne Pathogens if requested by administration. Perform other duties as assigned.
