Position Overview:
The Hospice Registered Nurse is responsiblefor pain management, symptom control in the delivery of care tohospice patients, as established by the plan of care, theinterdisciplinary team, attending physician and Hospice MedicalDirector.
Position Overview :
Ensures the timely and adequatedelivery of hospice services to the terminally ill patient andtheir family, operating within the plan of care as established bythe hospice team and attending physician. Assesses the total needsof the patient/family during regularly scheduled and after hourhome visits;
Documents:
assessment, identified problems, nursinginterventions, goals, and outcomes of interventions. Coordinatestotal patient/family hospice care under the supervision of theDirector and with the interdisciplinary team, the attendingphysician, and other providers; documents such coordination.
Collaborates with the interdisciplinary team in the development,review and revision of the clinical component of patient/familyplan of care, including exchange of information, review ofproblems, assessing effectiveness of interventions and documentingoutcomes. Ensures continuity of care between patient/family, teammembers, ancillary providers, long term and inpatient carefacilities, and the attending physician. Communicatespatient/family/caregiver needs, ongoing nursing assessment,interventions, goals and outcomes through the interdisciplinaryprocess. Provides and documents continuing education of the hospiceconcept of care to patients and their families/caregivers.
Supervises the delivery of patient care provided by Hospice Aides,Licensed Practical Nurses; ensuring compliance with the establishedplan of care and completing required documentation of supervision.
Facilitates the delivery of hospice services to patients residingin long term care facilities, documenting joint coordination ofcare with facility professional staff, and attending facility careplan meetings as appropriate. Participates in on-call rotation fordelivery of care after office hours, on weekends, and holidays.
Provides ongoing monitoring of patient appropriateness for hospiceservices and completes required documentation for certification andrecertification. Interfaces with the patient/family/caregiver, thehospice team, other health care providers in a respectful,professional and courteous manner. Maintains compliance withMedicare Conditions of Participation, Joint Commission standards,Hospice and Professional Standards of Nursing Practice, and agencyspecific policies and procedures. Supports community education ofhospice for growth and development of the program. Dispensesmedical supplies in a cost effective manner, as dictated by theplan of care and patient needs. Participates in qualityimprovement, utilization review, and infection control activitiesas requested. Attends appropriate inservices, and participates incontinuing education. Ensures accurate and current patient chartinformation by timely and consistent documentation. Maintains aprofessional, well-groomed appearance, adhering to the agency dresscode. Maintains sound privacy and security practices and preventsprivacy or security breaches. If breach occurs, takes correctiveaction.

Current license as a Registered Nurse in the State of AlabamaMinimum of one (1) year medical/surgical experience. Hospice/HomeHealth experience preferred. Valid CPR and Drivers license HHH Asan employer accepting Medicare and Medicaid funds, employees mustcomply with all health-related requirements in all relevantjurisdictions, including required vaccinations and testing, subjectto exemptions for medical or religious reasons as appropriate.
