Overview:

We are seeking a dedicated Registered Nurse Case Manager to join our healthcare team. This position offers the opportunity to work in a residential treatment setting on the shores of Smith Lake, providing care and support to our veterans in need.

Responsibilities:

• Collaborate with our clinical department and appointed VAMC to ensure comprehensive patient care
• Develop and implement individualized care plans for patients
• Monitor patient progress and update treatment plans as necessary
• Coordinate with interdisciplinary teams to optimize patient outcomes
• Provide patient education on treatment plans and self-care strategies
• Conduct assessments and evaluations to determine patient needs
• Maintain accurate and detailed patient records

Skills:

• Experience in hospital medicine, critical care, acute care, medical-surgical, residential settings
• Strong physiology knowledge and understanding of complex dual-diagnosis conditions
• Ability to work effectively in a Residential Dual Diagnosis environment
• Excellent communication and interpersonal skills
• Critical thinking and problem-solving abilities
