How you’ll contribute Are you passionate about guiding patients through their rehabilitation journey with clarity, care, and collaboration? We’re looking for a dedicated Case Manager to be a vital part of our interdisciplinary team, ensuring seamless care coordination and successful discharge planning. How you’ll contribute Complete orientation and ongoing competencies to support departmental excellence Collaborate across departments to resolve barriers to care or discharge Keep documentation on track—including the Individual Plan of Care (IPoC)—in line with CMS guidelines Schedule and lead meaningful family conferences to keep caregivers informed Collect and report performance improvement data to elevate care quality Strong organizational and interpersonal skills Knowledge of the rehabilitation process and age-appropriate care planning Qualifications and requirements: Current Registered Nurse or Social Work licensure or Healthcare professional licensure as Respiratory Therapist, Physical Therapist, Speech Language Pathologist or Occupational Therapist. Certification in Case Management or Rehabilitation Nursing preferred; for example, Commission for Case Manager Certification (CCM); Association of Rehabilitation Nurses (ARN) certification, American Case Management Association (ACM) or Board Certification in CM by the ANCC e.g.: RN-BC Minimum of 2 years social work or case management experience in an inpatient setting highly preferred Acute/rehabilitation hospital experience preferred.
