POSITION SUMMARY

The Nurse Care Management position oversees the operations and performance of the nursing team, specifically managing the roles of Nurse Care Coordinator, Nurse Care Managers for Advanced Care Programs. This position is crucial in ensuring that patients receive high-quality care while maintaining ethical standards and compliance with program guidelines.

PRINCIPAL DUTIES AND RESPONSIBILITIES
Leadership and Management:

o Provide leadership and direction to nursing staff, ensuring alignment with organizational goals and standards. o Monitor staff performance, provide feedback and facilitate professional development opportunities. o Conduct regular team meetings to discuss program updates, best practices and any challenges faced by staff.

Oversight:

o Ensure that all patient care activities comply with ethical standards and regulatory requirements. o Support nurse managers in assessing patient eligibility for programs and coordinating care management. o Oversee the collection and analysis of data necessary for program enrollment, reporting and performance improvement.

Collaboration and Communication:

o Act as a liaison between Centers of Excellence (COE) facilities, care managers and other stakeholders to ensure seamless patient care. o Foster positive relationships with external vendors and providers to enhance service delivery and patient satisfaction. o Communicate effectively with patients and their families, providing education and support related to treatment plans and program policies.

Administrative Duties:

o Review and approve coverage checks, claims inquiries and other administrative tasks related to patient care. o Participate in organizational meetings and contribute to strategic planning and resource allocation. o Identify issues and implement solutions to enhance operational efficiency and patient outcomes.

KNOWLEDGE, SKILLS AND ABILITY REQUIREMENTS

Associate or Bachelor of Science in Nursing required. Current, unencumbered RN license required. Minimum of 3 years of clinical nursing experience with at least 1 year in a leadership or management role preferred. Case Management certification (must be obtained within 2 years of hire). Basic proficiency in Microsoft Office (Word, Excel, Teams, Outlook). Strong organizational, communication and interpersonal skills.
