In collaboration with the multidisciplinary team, theSpine Nurse Navigator is responsible for supporting the patientthroughout the continuum of care, functioning as a patientadvocate, liaison, and educator. Key areas of responsibility
- Performs daily patient rounds during inpatient hospital stay;reinforces patient education regarding disease process andrehabilitation goals. This includes coverage for our Elective,Urgent, and Emergent Spine populations.
- Collaborates with theOrtho Trauma Nurse Navigator to support the Trauma Spine populationthroughout peri-operative and post-operative touchpoints.
- Servesas a liaison for the Spine patient population between the patientand surgeon, inpatient teams and appropriate outpatient services asdetermined by patient’s clinical needs.
- Performs pre-operativeand post transition outreach to identify problems and provide earlyintervention.
- Provides support to all Spine patients during anyReadmissions to the hospital as well as provides post-transitionoutreach after a return to the Emergency Department.
- Incollaboration with other healthcare team members, providesoversight and direction of patient continuous quality improvementmetrics, such as: patient experience, falls, readmissions,percentage of SNF referrals, and other quality metrics.
Participates in daily clinical progression rounds and weeklyprospective patient rounds.
- Participates in clinical quality careimprovement projects and Research, resulting in measured outcomesto optimize the patient experience.
- In partnership with NursingLeadership, recruits and promotes participation in patientengagement committees
- Provides perioperative education tooptimize patient outcomes
- Supports the preoperative andpostoperative patient in a holistic manner by engaging appropriateresources to meet their needs.
- Participates in workshops,seminars and community outreach programs to support and advance theBone and Joint mission.
- Participates in clinical research tosupport and enhance evidence-based nursing practice.
- Ensureproper Patient Reported Outcomes (PRO) captures on selected virtualplatforms. Qualifications Education
- Bachelor of Science inNursing
- Masters (Preferred) Experience
- Minimum 5 years clinicalnursing experience preferred
- Service Line and/or operationalexperience (Preferred) Licensure, Certification, Registration
- Active CT Registered Nurse license Knowledge, Skills and AbilityRequirements
- Demonstrates use of clinical skills and expertise inthe area of diagnosis, treatment, and psychosocial impact ofdisease.
- Demonstrates a commitment to own professional growth anddevelopment.
- Ability to communicate effectively with diverseaudiences including professional staff, patients, community, mediaand other service agencies.
- Demonstrates excellent customerservice skills including superior telephonic communications skillswith the ability to comprehensively assess patients’ physical andpsychosocial status as well as conduct complex medical historydiscussions over the telephone.
- Knowledge of adult learningcompetencies and patient teaching skills.
- Tolerance of ambiguityand the flexibility to respond to changing system needs.
- Demonstrated management skills reflecting self-motivation,initiative and fiscal responsibility.
- Skills and knowledge ofMicrosoft office tools (Word, Powerpoint, Excel).
