RN Home Care Case Manager Advanced Home Health Care Agency,INC 104 Park Road, West Hartford, CT 06119 Job Summary Advanced Home Health Care – Licensed, Medicaid and Medicare certified Agency, located in West Hartford, has an immediate opening for Behavior / Med-Surg Registered Nurse. As a RN with Advanced Home Health Care Agency you will provide patient evaluations, implementation of the plan of care and treatment to patients in their homes in order to reach clinical and functional goals.
Duties and Responsibilities:
Provides skilled nursing care to patients requiring professional nursing services in their homes. Observes patients for changes in condition, reports changes appropriately to the physician, Administrator/Clinical Director, and other members of the home care team. Initiates and maintains implementation of orders for medication and treatments as prescribed by the physician. Initiates and applies appropriate preventative, therapeutic, and rehabilitative procedures and techniques. Assists and instructs patients, family members or other patient representatives in the implementation of the care plan necessary to promote the patient’s health, safety, and independent living. Participates in case conferences with other members of the health care team. Participates in quality improvement/ assurance activities as assigned.
Minimum Requirements:
Current state license as a Registered Nurse.
Minimum of 2 year experience as a Registered Nurse in Home Health required.
Demonstrated knowledge and skill in current nursing practice.
Evidence of effective communication and organization skills Holds a valid driver’s license.
