Required Qualifications:

One year ofrecent experience as a Registered Nurse, including a minimum of sixmonths in a Home Health setting as an RN Proficient in OASISdocumentation Valid RN license in compliance with state regulationsBLS certification (AHA/ARC)

Preferred Qualifications:

Home HealthNursing Certification (RN-BC) Other certifications and licenses maybe required for this position

Summary:

A Home Health RegisteredNurse (RN) delivers direct nursing care to patients in their homes,ensuring comfort, stability, and effective management of theirmedical conditions. Home Health RNs assess patient conditions,develop care plans, administer treatments, and educate patients andcaregivers on disease management.

Essential Work Functions:

Assesspatient conditions and develop individualized care plans based onmedical needs and physician directives Administer medications,treatments, and wound care while monitoring patient responsesProvide patient and caregiver education on medication management,disease progression, and self-care techniques Monitor for changesin patient status, reporting concerns and adjusting care plans asneeded Coordinate with physicians, therapists, and other healthcareprofessionals to ensure seamless care transitions Document patientcare, interventions, and progress accurately and in compliance withregulatory standards Ensure compliance with infection control,safety, and home care best practices to protect patients andcaregivers Support families and caregivers, offering guidance andemotional support throughout the care process Maintaincommunication with case managers and interdisciplinary teams toprovide coordinated patient care Strong ability to completecharting and documentation requirements while meeting visit andproductivity goals Adhere to infection control, safety, andhealthcare regulations Perform other duties as assigned within theRN scope of practice and home health regulations

Required EssentialSkills:

Critical thinking, service excellence and goodinterpersonal communication skills The ability to read, write, &communicate in the English language is required Ability toread/comprehend written instructions, follow verbal instructions,and proficiency in PC skills Physical Abilities

• Remain in astationary position, move about, move equipment (50-100lbs),pushing, pulling, bending Interpersonal Skills
• To workeffectively with a variety of personnel (professional andancillary) to present a positive attitude and professionalismTechnical/Motor Skills
• Ability to grasp, fine manipulation,pushing/pulling and able to move about when assisting withprocedures and/or using department equipment. Mental Requirements
• Must be able to cope with frequent contact with the general public,customers, and meeting deadlines under pressure. Must be able towork under close supervision occasionally as well as workingwithout assistance from other personnel.
