Job Summary:
The licensed Case Coordinator – RN demonstrates expertise in ahealthcare setting in the assessment and treatment of patientsalong the continuum of care. This position develops and implementsdischarge planning within an interdisciplinary healthcare team andaddresses complex clinical care needs by identifying and removingbarriers that prevent optimum access to needed post-acute care.
This position also collaborates with the providers to identifydischarge barriers and delays in order to optimize the mostefficient use of acute care hospital days and prevent prolongedlength of stay. In addition, this position provides clinicalleadership to their healthcare teams and ensures daily goals arebeing met according to the patient’s plan of care.
JobResponsibilities:
- Assesses patient population in collaborationwith the Health Care Team, for appropriate level of care across thecare continuum; identifies risk factors such as clinical,psychosocial, and/or financial indicators and determines level ofcase coordination; facilitates involvement of support services topromote optimal clinical and financial outcomes.
- Develops andimplements discharge planning in collaboration with the Health CareTeam, by monitoring plans of care, coordinatingconsultations/referrals, identifying referrals for appropriatelevel of care, collaborating with patient and family and providingclinical information to community resources to provide efficientquality care.
- Collaborates daily with direct care providers toassess in-hospital stays and to facilitate post-hospitalizationservices, placement, and/or transportation.
- Models’ qualitypatient/customer service by always serving all customers,maintaining and functioning in a collaborative manner with othercaregivers to ensure quality patient care; maintaining awareness ofpatient/customer needs and responding to those needs; maintainingconfidentiality of all patient information.
- Provides ongoingassessment, including gathering comprehensive information to use indeveloping interventions and treatment strategies.
- Optimizesclient functioning through case management that facilitatescollaboration among providers to address the client’s biomedicaland psychosocial needs to better provide efficient, appropriate,and beneficial health care services to a client with multipleneeds.
- Advocates for the needs and interests of patientsincluding advocating for larger system change to improve access tocare and improved delivery of services.
- Educates patients,families, the community, and other professionals regarding diseaseprevention, impact of illness and disease progression, advocacy forbenefits, health maintenance, and adherence to treatment regimens.
- Maintains timely documentation of case management services whichreflect the patient and client systems’ pertinent information forassessment, interventions, and outcomes.
- Provides timelyassessments of needs for discharge planning for patients withprolonged length of stay.
- Identifies discharge delays throughoutthe hospitalization and develops countermeasures for potentialbarriers to discharge.
- Identifies lack of payer source and makesappropriate referrals to secure needed insurance coverage orproviders for post-acute care needs.
- Identifies opportunities todecrease length of stay, reduce readmissions and improve patientexperience through work in Lean huddles and participating ininterdisciplinary team meetings.
- Assess patient’s readiness fordischarge based on patient’s clinical status.
- Collaborates withproviders to ensure appropriate patients are placed at appropriatelevel of care throughout their hospitalization and documentationsupports patient’s present level of care.
- Performs other relatedduties as required.
Qualifications Licensure / Certification /
Registration:
Required Credential(s):
- Licensed Registered Nursecredentialed from the Connecticut Department of Public Healthobtained prior to hire date or job transfer date. PreferredCredential(s):
- Certified BLS Provider credentialed from theAmerican Heart Association (AHA).
- Certified BLS Providercredentialed from the American Red Cross.
- Case ManagementCertification preferred.
Education:
- Completion of a formalNursing program.
- Associate’s degree hires, Nursing BaccalaureateDegree (BSN) must be obtained within 6 years of hire.
WorkExperience:
- Three years of acute-care setting, diversifiedclinical nursing experience, including medical/surgical nursing.
- Case management, home care, and/or managed care experience stronglyconsidered.
- Previous experience in multiple healthcare settings(preferred). Knowledge, Skills, and Ability Requirements
- Demonstrate comprehension of medical terminology, natural historyof illness and general disease processes; identification of andreliance on educational resources to continuously improve clinicalpractice as a medical social worker.
- Excellent communication,negotiation and conflict resolution skills required.
- Knowledge ofcomputer applications preferred.
- Possesses ability to provideexpert verbal and written clinical documentation and consultationalong the continuum of care.
- Must be able to workcollaboratively, efficiently, and effectively withmultidisciplinary health care professionals to ensure a seamlesstransition of care for our patients and families.
- Ability tomulti-task and address multiple needs of healthcare team membersand patients/families.
- The ability to address complexpsychosocial needs by collaborating with community resources andaddressing barriers that prevent patients from optimizing theirhealth and quality of life.
- Ability to work in fast changinghealthcare environment.
- Must maintain CEU’s as required for RN,CCM and/or ACM licensure.
- Preferred bilingual English/Spanish.
