Position Summary:
An assessment nurse’s primary responsibility is to evaluate patients’ health status, needs, and eligibility for services. This involves gathering comprehensive information through various means, including physical exams, reviewing medical history, and engaging in interviews with patients and their families. Assessment nurses play a crucial role in developing and implementing care plans, monitoring patient progress, and collaborating with other healthcare professionals to ensure optimal patient outcomes. Prior knowledge of 120-day assessment required.
Physical Demands and Working Conditions
- Ability to lift/carry up to 50 pounds. 80 pounds of pressure may be placed on the associate through transfers using transfer belts or mechanical assists.
- Must be able to stand, walk, stoop and/or bend for periods of up to eight hours with breaks as provided for in the employee handbook.
- Possible exposure to communicable diseases, infectious diseases, and bloodborne pathogens.
