The licensed RN care coordinatordemonstrates expertise in a healthcare setting in the assessmentand treatment of patients along the continuum of care. Develops andimplements discharge planning within an interdisciplinaryhealthcare team, addresses complex clinical care needs byidentifying and removing barriers that prevent optimum access toneeded post-acute care. Works collaboratively with the providers toidentify discharge barriers and delays in order to optimize mostefficient use of acute care hospital days and prevent prolongedlength of stay. Provides clinical leadership to their healthcareteams and ensures daily goals are being met according to thepatient’s plan of care. Qualifications Education Graduate from anaccredited school of nursing, BSN Preferred Previous experience inacute care healthcare setting or in multiple healthcare settings.
Licensure, Certification, Registration Valid Registered NurseLicense with the State of Connecticut Case Management Certificationpreferred Language Skills Preferred bilingual English/SpanishKnowledge, Skills and Ability Requirements
- Demonstratecomprehension of medical terminology, natural history of illnessand general disease processes; identification of and reliance oneducational resources to continuously improve clinical practice asa medical social worker.
- Excellent communication, negotiation andconflict resolution skills required.
- Knowledge of computerapplications preferred.
- Possesses ability to provide expertverbal and written clinical documentation and consultation alongthe continuum of care.
- Must be able to work collaboratively,efficiently and effectively with multidisciplinary health careprofessionals to ensure a seamless transition of care for ourpatients and families.
- Ability to multi-task and address multipleneeds of healthcare team members and patients/families.
- Abilityto address complex psychosocial needs by working with communityresources and addressing barriers that prevent patient fromoptimizing their health and quality of life.
- Ability to work infast changing healthcare environment.
