Job Description /n /n /n The Registered Nurse (RN) isaccountable for providing a restorative healing experience for thepatient and the patients support systems in transformationalpartnership with the Clinical Manager and the interdisciplinaryteam. As an accountable professional, the Registered Nurseexercises autonomy (the right to undertake a specified action),authority (the power to implement action), and control (the abilityto enforce the action in an ongoing and consistent manner) in orderto ensure these excellent patient outcomes. Professionalaccountability is expressed in five areas: practice, qualityassurance, competence, research, and resource management. /n/n
Responsibilities:

- Patient Assessment
- Demonstratesautonomy in practice by performing a thorough and ongoing holisticnursing assessment, including appropriate admission assessment ofpatient and patient support systems.
Upon admission, the RNidentifies, prioritizes and documents current and potentialproblems in developing an individualized plan of care thataddresses needs and concerns. Develops appropriate interventionsand desired outcomes for key problems with the goal of positive andeffective patient outcomes. Evaluates problems based on patientcondition and per policy. /n/t
- Patient Care/Discharge Planning
- Nurses are accountable for their own practice in the coordinationof care.
The RN will collaborate with patient, patient supportsystems, and appropriate health care team members, to meet thepatients present needs, discharge needs and outcomes identified forkey problems. Implements a plan of care upon admission or within 24hours of admission. Recognizing that interdisciplinarydecision-making is essential, the RN initiates interdisciplinaryreferrals upon admission and as needed. The RN facilitates theevaluation, revision and documentation of the patients plan of careon a regular and on-going basis. /n/t
- Teaching
- The RN isempowered to exercise independent judgment and critical thinkingskills to process information on the plan of care to provideindividualized, ongoing teaching to patient and patient supportsystems throughout the continuum of care. Teaching is a vitalcomponent of nursing care. The RN uses current, evidence-basedpractice when teaching patients and support systems. /n/t
- Communication
- As a professional, the RN demonstrates mutualrespect and collaboration when communicating with theinterdisciplinary health care team.
Communication is a two-wayprocess in which the RN mindfully utilizes effective communicationskills such as active listening, relationship building andassertive communication techniques. The RN conveys a strong senseof advocacy and protects patient rights. The RN strives to createhigh performance teams by providing the teams with an overallpositive, inclusive sense of support. /n/t
- Treatment/ Procedures
- As the essential provider of healthcare, the RN performs alltreatments and procedures in accordance with establishedevidence-based policies, procedures and patient care Standards. TheRN collaborates with the MD and all members of the health care teamin the performance of these tasks. /n/t
- Documentation The RNaccurately, efficiently, and in a timely fashion, documentspertinent patient information Utilizing the nursing process on allrequired forms or fields/ screens to ensure that patientinformation is collected and communicated effectively to ensureexcellent patient outcomes. /n/t
- Medication Administration The RNprovides all aspects of the medication process including:assessment, administration, monitoring, and evaluation of thepatients response to the medication. The RN teaches the patient andthe patient support system regarding the safe use of medicationsand initiates referrals to Pharmacy as needed. /n/t
- QualityPerformance and Improvement (QPI)
- Actively participates in theQPI process to achieve excellent patient outcomes and improvedprocesses.
Participation in QPI includes the development of the QPIplans by identifying actual or potential problems, collecting data,analyzing the results, implementing corrective action plans andsustaining improvements. The RN actively participates inmaintaining and improving the Culture of Patient Safety. /n/t
- Delegation to and Development of Co-workers
- The RN accuratelydelegates aspects of patient care to the LPN, CNA, and otherancillary personnel according to their Scope of Practice, jobdescription, individual competence and patient needs.
The RNsupervises the provision of this care by clearly communicating theright task in the right circumstance to the right individual, whilebeing available to provide the right direction and supervision. TheRN orients, precepts, mentors, teaches and fosters the developmentof co-workers from Novice to Expert. /n/t

Education:

- Graduate of anaccredited school of nursing, currently licensed in the State ofNew York, or permittee.
Membership in a professional organizationsuggested. /n /n/n
Experience:
/n/n /n/t
- Minimum of one yearMed/Surg Experience preferred. Minimum of 2 years critical care oremergency room experience preferred. /n/t
- Basic knowledge ofnursing theory and practice.
Knowledge of appropriate technologyand practice standards for area of responsibility. Demonstratessound clinical skills; knowledge of Hospital structure, objectivesand standards of practice, positive communication skills. /n /n/n
Training/License:
/n/n /n/t
- New York State Registered Nurselicense required, or permittee. /n/t
- BLS, ACLS and PALSCertifications required.
TNCC, ENPC and CEN preferred. /n /n/n
