Home Health and Hospice RN Position Available In Western Connecticut Planning Region, Connecticut

Are you an experienced Home Health or Hospice RN? We have a need for a weekend Home Hospice RN for the Fairfield County CT area. The schedule is 24 hours, AM shift. We would welcome the chance to to speak with you about this opportunity. Must be skilled in pain and symptom management. Able to triage patient needs and make patient visits as appropriate. Provide skilled and supportive nursing care to patients and families. Perform assessment of patient and family, through observation, interviews, and analysis of records Respond to physical, emotional psychosocial, environmental, needs. Identify patient and family needs for other home health care services and makes referrals through agency-designated channels Identify signs and symptoms of the disease and explains diagnosis and the nature of palliative care to the patient and family as appropriate Utilize appropriate nursing interventions to alleviate pain and discomfort in accordance with the plan of care. Supports decision making by the patient and primary caregivers Minimum of two years of nursing experience, preferably in medical/surgical nursing required Valid Clean Driver’s License Required Home Health Care and/or Hospice / End of life care experience preferred Active client contact within past three years, preferred. Certified Hospice and Palliative Care Nurse (CHPN) preferred
