What You’ll Do:

As an MDSCoordinator / Nurse Assessment Coordinator, you will complete andassure the accuracy of Minimum Data Set (MDS) assessments for allresidents. The MDS Coordinator / Nurse Assessment Coordinatorcontributes to personalized resident care plans and ensures thecapture of clinical reimbursement for services provided.

KeyResponsibilities:

Determine Patient Driven Payment Method (PDPM)and expense associated with a potential admission Participate inadmitting prospective residents by assessing their nursing needsand determining appropriate clinical reimbursement levels Completeand assure the accuracy of the MDS process for all residentsMonitor Case Mix Index (CMI) scores, looking for potential risksand/or changes that may affect reimbursement Ensure the highestlevel of revenue integrity and compliance to all state and federalregulations for MDS completion and coding conventions Collaboratewith interdisciplinary teams to ensure accurate data collection forassessments Provide insights and ongoing education to facilitystaff and leaders If you are passionate about ensuring exceptionalresident care through accurate, detailed assessments anddocumentation, consider this exceptional opportunity. What You’llBring:

Qualifications of a MDS Coordinator / Nurse AssessmentCoordinator include: Valid state RN nursing license Advanced degreeor certification preferred Direct care in a long-term care setting,MDS Coordinator, Clinical Reimbursement Specialist or NurseAssessment Coordinator experience preferred Knowledge of state andfederal regulations governing the MDS, Electronic Medical Record(EMR), PDP, MDS 3.0, Medicaid and Medicare requirements helpfulInterest in the nursing needs of the aged and the chronically illwith the ability to work with both Deadline driven, detail-orientedindividual with strong organizational skills, analyticalcapabilities and the ability to make decisions independentlyExcellent written and verbal communication and interpersonalabilities Ability to work effectively and influence others in amultidisciplinary team environment Tier1 -: We Hire for Heart!
