General Statement of Duties:
Responsible for completion, co-coordination and management of the Minimum Data Set (MDS) and care planning processes for the skilled nursing residents, regardless of payor source. This includes all Medicare Part A, commercial insurance and managed care of our short-term patients. Completion of all other required MDS and care planning for our long-term residents as required.
Qualifications and Experience:
Requires a licensed practical nurse with a current Connecticut State license and a graduate of an accredited school of Nursing. Individual must have experience in medical/surgical or geriatric nursing. Must have excellent interpersonal and organizational skills to deal with all levels of people inside and outside the organization. Must have knowledge of the MDS and care planning skills. Must be flexible to work a varied time schedule. Must possess a working knowledge of OBRA regulations as they relate to Skilled Nursing facilities; as well as Medicare Reimbursement under Patient Driven Payment Model “PDPM”.
