Travel Performs an age specific plan of care for adesignated group of patients using the nursing process ofassessment, diagnosis, outcome identification, planning,implementation and evaluation of patient care. Collaborates withphysicians and other health team members in coordinating andimplementing procedures and treatments. Uses leadershipskills/clinical judgment in coordinating patient care anddirecting/delegating activities of the patient care unit team.
Responsibilities:
- 1.
Assesses the patient’s physiologic healthstatus Conducts patient interview, explains policies and proceduresto patient/significant others, reviews patient’s chart and answersquestions correctly and courteously; Assesses gastrointestinal,cardiovascular, respiratory, renal and neurological health status;Determines mobility, sensory deficits, prostheses use, and skincondition and adjusts plan of care, as appropriate; Assesses levelof pain and pain management; Communicates/documents patient’sphysiologic health status and plan of care.

Assesses patient’spsychosocial health status Elicits perception of medical/nursingcare and the expectation of care; Determines coping mechanism,knowledge level, and ability to comprehend; Identifiescultural/ethnic requirements to reduce anxiety and ensure patientsatisfaction; Communicates and documents psychological status andcare plan; support group, counseling services, social services.
- 3.Identifies patient outcome Develops criteria for measurement ofpatient outcomes; Identifies actual/potential patient problems;Identifies patient’s need for teaching based on psychosocial anddevelopmental assessment; Develops patient outcome statement (s)and establishes individualized patient goals.
- 4. Formulates theplan of care and establishes the priorities necessary to achieveexpected outcome Identifies care activities and establishes thepriorities necessary to achieve expected outcome; Coordinates thecost-effective use of supplies, equipment and medication to achieveexpected outcome; Documents the plan of care and collaborates withphysicians and other health team members.

Implements the planof care Maintains constant vigilance over patients care to ensurethat safety precautions/needs are followed. (Side rails up, calllights and bedside stand within reach, etc.); Exercisesprofessional skills related to the plan of care; Reassessespatients as needed and appropriately revises plan of care;Correctly administers prescribed treatments; Correctly usesequipment necessary for patient care; Provides emotional support topatient and significant other; Applies scientific principles inperforming procedures; carries them out safely, timely andefficiently; Makes accurate observations of patient’s conditionsduring treatments/procedures. Reports and records same asappropriate; Keeps accurate documentation of patient’s treatment,activity and condition, as well as patient’s responses to medicaland nursing interventions; Uses appropriate methods ofdocumentation according to departmental policy; Acts rapidly andeffectively during any emergency situation, managing self, patientsand other employees; Provides a calm, quiet, restful atmosphere.
Communicates effectively with the patient’s family or caretakers;Participates in planning for discharge and coordinates referrals,as appropriate; Provides discharge instructions to patient andsignificant other.

Evaluates care provided for patient outcomeDemonstrates ability to measure effectiveness of care provided anddocuments same; Performs variance analysis related to outcome datafor performance improvement; Designs, implements, and evaluatessystems to improve care in Unit; Keeps accurate documentation ofpatient’s treatment, activity and condition; Uses appropriatemethods of documentation according to departmental policy.

Collaborates with other care team members in planning and carryingout treatment regimen; provides direction to other members of thecare team Collaborates with the appropriate physician on patient’splan of care; Accurately interprets and implements treatmentregimen as prescribed by the Physician; Assists the Physicianduring treatments and/or diagnostic procedures; Keeps the PatientCare Manager/designee and/or physician, abreast of changes inpatient’s condition and/or treatments, as appropriate; Usesclinical judgment in delegating assignments to the Patient CareAssociate, Support Care Associate and Clerical Service Associate inproviding patient care, and ensures that assignments are completedin a timely fashion.

Performs grade I-IV Decubitus CarePerforms preventive skin care measures Applies simple dressings,maintaining principles of aseptic technique; Applies warm & coldcompresses, consistent with facility procedure; Performsirrigations, consistent with Facility procedure; Performs otherprocedures related to skin care, as necessary.

Administersmedications correctly and safely Correctly identifies medication:action; dosage; side effects and implications for the patient;Meets standard on medication administration examination;Demonstrates preparation of local solutions; Demonstratespreparation of I.V. drips; Administers and documents medicationcorrectly; Administers IVP medication as per department policy andprocedure; Educates patients and significant others related to drugand food interactions.
