The ideal candidate will be responsible for coordinating patient care, ensuring effective utilization of resources, and facilitating communication among healthcare providers. This role is crucial in managing patient cases across various settings, including ICU, Emergency Room, and clinics, particularly within Level I and Level II Trauma Centers. The Registered Nurse Case Manager will play a vital role in enhancing patient outcomes through comprehensive case management. Duties Assess and evaluate patients’ needs to develop individualized care plans. Coordinate and manage patient care across multiple disciplines and settings. Utilize medical terminology effectively to communicate with healthcare professionals. Monitor patient progress and adjust care plans as necessary. Facilitate communication between patients, families, and healthcare teams. Ensure compliance with utilization management protocols and standards. Provide education to patients and families regarding treatment options and care plans. Document all case management activities accurately in the patient’s medical record. Collaborate with interdisciplinary teams to optimize patient care pathways. Qualifications Current Registered Nurse (RN) license in the state of practice. Proven experience in case management within a clinical setting. Strong knowledge of medical terminology and healthcare processes. Experience in utilization management preferred. Background in emergency medicine or critical care (ICU) is highly desirable. Familiarity with Level I and Level II Trauma Center operations is a plus. Excellent communication skills, both verbal and written. Ability to work collaboratively within a multidisciplinary team environment. Strong organizational skills with attention to detail.
