JOB PURPOSE
To provide and coordinatethe provision of skilled nursing care under physician supervision,through competent application of nursing process.
KEYRESPONSIBILITIES
- Provides competent nursing care & interventionsto home health patients according to state and industry andprofession
- Supervises care provided by Home Health Aides,Licensed Practical Nurses (LPN) assigned to the care team(completing supervisory visits of HHAs as required)
- Helpsmaintain continuity of care by collaborating with theinterdisciplinary team and attending physician to develop,integrate, and manage the patient care plan of care, makingreferrals to other disciplines as needed
- Demonstratesintermediate knowledge of home care and competency in disciplinefor specific patient care skills, required for the provision ofcare
- Conducts field visits to (1) evaluate & initiate start ofcare and (2) conduct clinical outcomes monitoring, follow-up andcare performance improvement initiatives
- Utilizes acceptedcompany teaching materials in patient/family education
- Assesses,monitors, documents and reports progress of patient’s health andcondition using required documents via clinician preferredtechnology, and within established timelines
- Completesdocumentation timely, accurately, and at the point of care,according to industry standards (e.g., OASIS assessments,SOC/Admissions, Recertifications, ABN’s, subsequent visits,physician orders, care coordination, etc.)
- Honorspatients/residents’ rights to be notified in advance of care,treatment and services to be provided and works with patient teamto adhere to physician-approved care plan. Ensures equitabletreatment, self-determination, individuality, privacy, property andcivil rights.
- Complies with corporate compliance program
- Meetsagency productivity standards and utilizes time and resourceseffectively and efficiently
- Participates in center/agency surveys(Licensure/Joint Commission), mandatory in-services, caseconferences, staff meetings, and programs (e.g., PerformanceImprovement Program) as required
- Follow established safetyregulations and procedures, and report job-related functions/tasksthat involve occupational hazards, as necessary
- Maintains currentlicense, certification, and clinical records to meet federal, stateand agency regulations and guidelines
- Ability to beself-directed, prioritize, manage time appropriately, and meetoutcomes and strategic goals by targeted deadlines.

MINIMUM EDUCATION REQUIRED
- Nursing degree froman accredited college or university, or graduate of anaccredited/approved RN program.
MINIMUM EXPERIENCE REQUIRED
- Minimum of one (1) year of RN experience
- Valid driver’s license
- Reliable transportation
- Liability insurance to complete homevisits
MINIMUM LICENSURE/CERTIFICATION REQUIRED BY LAW
- Current,active, and unrestricted Registered Nurse (RN) licensure in stateof practice.
- Current CPR certification.
ADDITIONALQUALIFICATIONS
(Preferred qualifications)
- Bachelor of Science inNursing (BSN) preferred
- Acute care experience with adult or agingpopulations
