The Clinical Nurse Manager will work with our Director of Nursing and Director of Operations to oversee the care provided to our clients. A typical day may include performing new client assessments, home infusions, overseeing the care of existing clients (personal care and skilled), and care coordination with skilled service organizations. Compassion, patience and an outgoing personality are qualities that will build trust and a level of comfort with clients and their families. The Case Manager will work primarily in the field with some administrative office responsibilities as well. Provide care as defined within the scope of the state’s nurse practice act Make the initial evaluation visit and regularly reevaluate the client’s nursing and/or personal assistance needs
Initiate appropriate preventive and rehabilitative nursing procedures and counsel client and family on nursing related needs
Administer agency policies and procedures
Assist with assessing the competency of staff to promote quality, continuity and safety of client care
Promote home care referrals in the health care community
Registered nurse with current unencumbered license to practice professional nursing in the state the job is located Minimum of three (3) years of documented experience required. Two (2) or more years of clinical experience with one (1) year experience in a home health agency or case management preferred
Understanding of regulatory compliance local, state and federal laws, Joint Commission, OSHA and HIPAA
Flexibility and good time management skills A MUST
Ability to travel locally
