MDS Coordinator RAM Recruiting Starke, FL 32091 MDS Coordinator – Ensure Excellence in Resident Care! A leading Nursing Home in your area is seeking a highly organized and detail-oriented professional to join our team as an MDS Coordinator . This role is essential in ensuring that our residents receive the highest quality of care through accurate assessments and comprehensive care planning.
Key Responsibilities:
Coordinate and oversee the MDS assessment process, ensuring accurate and timely completion. Conduct resident assessments , develop individualized care plans , and ensure appropriate services are provided. Collaborate with interdisciplinary teams (nursing, rehabilitation, dietary, etc.) to ensure assessment accuracy and care quality. Monitor and analyze resident outcomes , using data to drive continuous improvement in care and services . Educate and train staff on MDS processes and regulatory requirements.
Qualifications:
Active RN license with experience in long-term care . Strong knowledge of MDS assessment processes and regulatory compliance. Excellent organizational, analytical, and problem-solving skills. Strong communication and interpersonal abilities. Ability to work collaboratively with residents, families, and healthcare teams .
