Nursing Full Job Description Job Summary:

We are seeking an experienced, highly organized and detail-oriented MDS Coordinator to join our healthcare team. As an MDS Coordinator, you will be responsible for coordinating and overseeing the Minimum Data Set (MDS) process in compliance with federal and state regulations. Your role will involve collaborating with healthcare professionals to ensure accurate and timely completion of assessments and documentation.

Duties:
  • Coordinate and oversee the MDS process, including scheduling and conducting assessments
  • Collaborate with interdisciplinary teams to gather information for accurate completion of MDS assessments
  • Ensure compliance with federal and state regulations related to MDS assessments
  • Review medical records and conduct interviews with patients to gather necessary information
  • Analyze patient data and develop comprehensive care plans based on individual needs
  • Provide education and training to staff on MDS documentation requirements
  • Participate in utilization review and medical management activities to optimize patient care
  • Maintain accurate and up-to-date records of MDS assessments and related documentation
Qualifications:
  • Bachelor’s degree in nursing or related field (preferred)
  • Valid nursing license (RN/LPN)
  • RAC Certification a Plus
  • Experience in hospice care, nursing home, primary care, or hospital setting
  • Strong knowledge of physiology and medical terminology
  • Familiarity with medical records systems and utilization review processes
  • Excellent attention to detail and ability to accurately analyze patient data
  • Strong communication skills, both written and verbal
