We are offering a great opportunity for a Care Manager to work with our cutting-edge in-home company.
Role Description:
We are seeking a motivated and experienced Care Manager with a strong background in cardiology, Nephrology, Infectious Disease and pulmonology. You will coordinate and manage the care of patients with cardiovascular and respiratory conditions, ensuring high-quality and comprehensive healthcare services. This kind of role is best suited for those with strong nursing backgrounds or Foreign Medical Graduates/International Medical Graduates, Medical Assistant, LPN, EMT/Paramedic.
Responsibilities:
- Serve as the primary contact for patients, providing support and guidance online
- Conduct thorough assessments of patients’ medical history, symptoms, and diagnostic results with emphasis of cardiology and pulmonology
- Collaborate with specialty physicians to develop personalized care plans
- Facilitate communication between the patient and healthcare team
- Monitor and evaluate progress of patients’ care plans and make necessary adjustments to optimize treatment, as directed by physicians
- Assist patients in accessing specialized cardiology and pulmonology services as needed.
- Educate patients and families about conditions, treatment options, and self-care strategies
- Advocate for patients’ rights and preferences related to their care.
- Maintain accurate documentation of patient interactions, assessments, and progress notes
- Stay current with advancements in cardiology and pulmonology treatments, research, and best practices
Qualifications:
- Bachelor’s degree in nursing or related healthcare field. Foreign Medical Graduate or International Medical Graduate strongly preferred. RN, LPN, MA, EMT/Paramedic with extensive clinical monitoring skills also considered.
- In-depth knowledge of cardiology and pulmonology terminology, disease processes, and treatments
- Excellent communication and interpersonal skills
- Strong organizational and time management abilities
- Ability to work independently and prioritize tasks in a fast-paced environment
- Proficient in using electronic medical record (EMR) systems and other relevant software
- Current certification (e.g., Certified Case Manager, Certified Rehabilitation Counselor, EKG, ACLS, BLS) is a plus
