Home Health RN The Roads Home Health Port Charlotte, FL Objectives:
The RN is a licensed professional who provides nursing care in compliance with healthcare standards, state/federal laws, and company policies. The RN uses their skills to meet patient needs and achieve measurable outcomes. This role involves exposure to blood as per OSHA standards.
Reporting Structure:
The RN reports to the Director of Nursing (DON) or a designated RN. During facility assignments, the RN also coordinates with the facility’s supervisor and adheres to its policies.
Key Responsibilities:
1.
Patient Care:
- Provide skilled nursing interventions based on patient needs and care plans.
- Develop, implement, and revise the nursing plan of care in collaboration with physicians and the DON.
- Use a holistic approach to assess and address patient needs.
- Ensure compliance with care standards and update the plan as needed.
2.
Care Coordination:
- Communicate effectively with patients, physicians, and healthcare team members.
- Act as a liaison to ensure coordinated care and supervise paraprofessional staff.
- Maintain confidentiality and ensure the availability of necessary supplies.
3.
Teaching and Training:
- Educate patients and families on health maintenance and care goals.
- Train and guide healthcare team members, including paraprofessional staff, as required.
4.
Documentation:
- Accurately document care, observations, and interventions.
- Submit required documentation in a timely manner per agency policies.
- Ensure accurate transcription and implementation of physician orders.
5.
Professional Conduct and Development:
- Participate in quality improvement activities.
- Attend and lead training sessions and maintain professional credentials.
- Adhere to dress code, demonstrate reliability, and uphold professional standards.
Qualifications:
- Graduate of an accredited nursing program.
- Current RN license and CPR certification.
- At least one year of experience in acute care, home health, community health, or outpatient care within the past two years.
- Valid driver’s license, reliable transportation, and auto insurance.
- Strong communication and leadership skills.
Physical and Language Requirements:
- Ability to read, write, and communicate effectively in English; additional languages are a plus.
