POSITION DUTIES

•Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans

MINIMUM REQUIRED QUALIFICATIONS

•FL/compact RN license; BLS; 1+ year recent acute hospital case management experience

PREFERRED QUALIFICATIONS

•Interqual & Meditech experience

LENGTH OF ASSIGNMENT

•13 weeks

SHIFT / HOURS PER WEEK

•8A-4:30P. Every other weekend & holidays in rotation.
