Job Summary:
We are seeking a dedicated and compassionate Registered Nurse – Home Health to provide exceptional patient care in a home setting. The ideal candidate will possess strong clinical skills and experience in various healthcare environments, including PICU, ER, and critical care. This role requires a commitment to delivering high-quality nursing care while ensuring the comfort and well-being of patients in their homes.
Key Responsibilities:
Conduct comprehensive patient assessments to develop individualized care plans. Administer medications and treatments as prescribed by physicians. Monitor and document patient progress, maintaining accurate medical records. Provide education and support to patients and their families regarding health management. Collaborate with interdisciplinary teams to ensure coordinated care. Assist with activities of daily living (ADLs) as needed. Respond promptly to patient needs and changes in condition. Ensure compliance with safety protocols and infection control measures.
Qualifications:
Active Registered Nurse (RN) license in Florida. Associate or Bachelor’s degree in Nursing (ADN/BSN) required. Minimum 1 year of nursing experience required. Minimum 1 year of case management experience required. Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certification (if applicable). Strong background in patient care, particularly in home health settings. Experience in critical care environments such as Level I Trauma Centers or inpatient facilities. Proficient in managing medical records and documentation practices. Ability to work independently while demonstrating excellent decision-making skills. Effective communication skills for interacting with patients, families, and healthcare teams. Familiarity with various medical equipment used for home health nursing.
