Job Description

MDS – Full time – in Office Desoto Health 475 Nursing Home Drive, Arcadia, FL 34266 ACCOUNTABILITY

OBJECTIVE

Supervise and coordinates all MDS assessments and care plans for residents within the facility.

BACKGROUND QUALIFICATIONS

Current license by the state Board of Nursing Must be RN Experience in long term care preferred Computer skills necessary

KEY RESPONSIBILITIES

Complete the MDS as outlined CMS and the facilities’ policy and procedure. Responsible for informing all care plan team members of when a care plan meeting is due and ensuring that all team member complete their portion of the MDS and appropriate care plan. Notify resident and their responsible party of care plan meeting. Complete trigger legend CAAS protocol. Develop and/or revise resident care plan quarterly and with any significant change in condition. Periodically compare care provided with care plan to ensure that residents are receiving services consistent with the plan of care. Responsible for admission and quarterly assessments for all residents, to include MDS, triggers, CAAS, fall risk assessment, pressure sore assessment, elopement assessment and side rail assessment. Coordinate all care plan meeting and inform DON of any team member who is not participating in the care planning process according to policy. Responsible for accurate and timely completion and submission of MDS according to Medicare/Medicaid guidelines. Responsible to initiate and maintain the Resident Assessment and Care Plan Schedule ensuring all dates for MDS and CAAS completion are met in accordance with guidelines. Provide copy of schedule to Business Office Manager, Rehabilitation Department and Corporate Accountants at the end of each month. Ensures residents’ rights are respected. Maintains effective communication amongst departments so assessments and care plans can be kept current when changes to the residents’ condition occurs. Monitors resident care during the assessment process and reports concerns to the DON. Assists with the pre-admission screening process to estimate the potential resident’s PDPM levels, RUGs group, as needed. Other duties as assigned.

Working Conditions Works inside. Physical Effort Lifting of 50lbs. Maximum with frequent lifting of objects weighing up to 25lbs. Frequent reaching, handling and fingering of instruments, equipment, records and reports. Long period of time at the computer. Standing, stooping, bending occurs when conducting assessment of residents. Physical Environment Clerical perception is necessary to prepare records and charts and to organize training programs. A preference for scientific and technical activities for understanding and responding to medical problems and concepts. Ability to interpret operating policies and procedures and to review work performance in determining conformance to these recognized standards. Is around bio-hazardous waste and blood-borne pathogens. Must have ability to assess residents and have good powers of observation. Sensory Attention Talking and hearing essential to instruct and supervise nursing personnel. Near visual acuity required to detect changes in condition of residents. Color vision for perceiving changes in residents skin color and colors of medications and solutions. Must have good sense of sight, smell and hearing to conduct proper physical assessments. Mental Stress Family and resident concerns can cause extreme discomfort. Must have compassion and ability to properly handle emotional situations. Deadlines for production of MDS/RAPs and care plans can cause long hours and stress to meet state and federal guidelines. I have read the above job description and understand the requirements. Nursing License :

Must be Registered Nurse (RN) . An RN is generally preferred, especially for facilities with more complex cases.

Experience :

Experience in long-term care or skilled nursing settings. Experience with MDS 3.0 assessments and care planning is usually required or preferred.

MDS Knowledge :

Strong understanding of MDS assessments , RUG-IV or PDPM , and federal regulations (CMS guidelines). Familiarity with Resident Assessment Instrument (RAI) manual and tools.
