Current Florida RN license Current Florida RN license 3+ years of clinical experience in a hospital setting Experience in two or more of the following: home health care,rehab, utilization review, discharge planning and casemanagement Experience with the Healthcare Industry (Managed Care) Experience collaborating with Medical Doctors, Managers, Co-workersand other departments to reach a resolution/decision regardingcare Experience with Case Management –Plan, implement, coordinate andmonitor Case Management activities that focus on acute andnon-acute services, outpatient services and/or community resourcesfor all lines of business Proficient in MS Word and Excel Job duties include but are not limited to the following: Engage, communicate with and patients onsite and telephonicallywith a focus on health coaching, patient education and coordinationof care for members. Improve the health and efficiency ofpatient-centered care Plan, implement, coordinate and monitor Case Management activitiesthat may include acute and non-acute services, outpatient servicesand or community resources for all lines of business Collaborate with Medical Directors, Managers, Co-workers and otherdepartments. Provide education and tools to promote self-management. Facilitate access to resources to meet member?s needs Promote healthy lifestyles, assist in strengthening thepatient-physician relationship, encourage behavior and lifestylechanges to realize a better quality of life, for individuals withidentified chronic conditions Serve as a point of contact for Plan Members (including ACOmembers) to assist in navigating them through the continuum,educate regarding benefits, identify candidates for Care Programsand provide information pertaining to the network and networkaccess. Participate in weekly case review to learn and share opportunitiesto improve outcomes.
