Job Summary Baptist AgeWell is seeking a compassionate full-time Nurse PractitionerHomecare to join our team. This is a primary care role where you will visit patients at home or in other settings. You must be comfortable and have experience working with adults with chronic medical conditions in a homecare setting. Good bedside manner is key for success in this role. You must possess the interpersonal skills to work with patients and families during a pivotal time. What You Can Expect This Nurse Practitioner will treat patients in their residence, home, ILF, or ALF. We provide Epic EHR System. A collaborative team with 4 other Advanced Practice Providers with Medical Assistants and support staff. Established geriatric patient panel with some palliative care patients. You will treat 6 to 7 patients a day. Hours are Monday through Friday from 8 AM to 5 PM. Call schedule is 2 times per year. The patient panel is aslo fed from Baptist Medical Center Nassau All About the Benefits Compensation is $114K to $140K+ based on experience with the ability to increase your compensation based on our productivity model.
