Psych Nurse – Duval/Volusia/Daytona Bravery Home Healthcare Jacksonville, FL 32202 Job Overview Bravery is a new, employee centric skilled home care company in Northeast Florida. As we open our doors to clinical care, we are looking for those not scared of the journey, passionate about the outcome, and inspired to be more in the roles they fill. Responsibilities Conduct comprehensive mental health assessments, including evaluation of cognitive, emotional, and behavioral status, alongside monitoring vital signs to detect clinical changes. Develop and implement individualized, evidence-based care plans in collaboration interdisciplinary team members. Educate patients and family members on diagnosis, treatment plans, medication adherence, symptom management, and coping strategies to promote recovery and self-management. Provide psychosocial support and guidance for managing psychiatric symptoms, pain, and co-occurring medical conditions. Maintain thorough, timely, and compliant documentation of patient progress and interventions using electronic health record (EHR) system. Skills Strong understanding of pain management techniques. Ability to take vital signs accurately and assess patient conditions effectively. Excellent interpersonal skills with the ability to communicate effectively with patients, families, and healthcare team members. Strong organizational skills with attention to detail in documentation and patient care. Proficient in post-operative and post-acute inpatient care. Experience in administering injections safely and effectively. Knowledge of tube feeding procedures. Ability to assess and monitor vital signs accurately. Familiarity with ostomy care practices.
