The qualifications: Current and experienced liaison working directly with or in Highlands County area hospital(s) and/or skilled nursing rehabs / clinical settings (prefer at least 2 years) Have been in the local Highlands area Florida healthcare market for at least 2 years Good relationship builder, with passion for helping seniors and families Case Manager, Nurse Case Manager, Social Worker background a plus The responsibilities: Obtaining referrals and handling case load with Case Managers Working with both new and established referral sources Meet directly with seniors and families in clinical and home settings
