Description
JOB SUMMARY
Under general supervision, and according to established polices and procedures, responsible for the provision of specialized clinical pharmacy services to patients, nurses and medical staff. Collaborates with other health care professionals in the research, development and implementation of critical pathways and policies that have a medication use component. Practice involves the components of expert practitioner, educator, consultant, researcher, and clinical leader. Responsible for performing job duties in accordance with the mission, vision and values of Tampa General Hospital.
Primary Duties:
Considers patient age and clinical condition as the hematology & oncology pharmacotherapy expert for Muma Children’s Hospital.
Develops EPIC Beacon protocols and optimizes Children’s Oncology Group therapy plans along with the multidisciplinary care team.
Supports clinical pharmacists and multidisciplinary care team members with provision of pharmaceutical care through direct interaction and formal educational activities.
Maintains expertise in areas of practice.
Educates pharmacists, technicians, students and other providers on relevant pharmacotherapy concepts and practice updates.
Disseminates drug information to patients, physicians, nurses, and other health care professionals.
Qualifications
Required:
Pharmacy degree (Pharm. D or Ph.D.) from an accredited college of pharmacy and completion of an advanced residency, fellowship, or equivalent experience.
Current license as a Registered Pharmacist in the state of Florida.
Preferred:
Completion of a Post-Graduate Year 2 Residency (PGY2) OR completion of a Post-Graduate Year 1 Residency (PGY-1) and three years of experience as a clinical pharmacist, including providing pharmaceutical care and performing pharmacotherapeutic monitoring for at least 50% of time in the specialty area.
Current Basic Life Support (BLS) Certification through the American Heart Association (AHA)
Advanced Cardiovascular Life Support (ACLS) or Pediatric Advanced Life Support (PALS) Certifications through the American Heart Association (AHA) within one year of hire for Pharmacists required to respond to adult or pediatric cardiopulmonary arrest events
