Role:

We are seeking a compassionate and experienced PRN Bilingual (Spanish/English) Registered Nurse Field Case Manager to join our home health team serving the Indian River and Fort Pierce area. In this role, you will provide skilled nursing care and case management for Medicare patients in their homes, using your language skills to deliver culturally sensitive and patient-centered care. You’ll be responsible for accurate and timely OASIS documentation, coordinating with interdisciplinary teams, and helping patients achieve the best possible outcomes in the comfort of their homes.

Key Responsibilities:

Conduct initial and follow-up assessments (including OASIS) for Medicare patients. Create individualized plans of care and coordinate services with physicians and other care team members. Deliver direct skilled nursing care and patient education. Communicate clearly in both Spanish and English to ensure understanding and compliance. Monitor patient progress and update care plans as needed. Maintain timely and accurate documentation in compliance with Medicare, state, and agency standards. Educate patients and caregivers on condition management and prevention strategies. Coordinate with intake, scheduling, and internal support teams. Maintain patient confidentiality and comply with HIPAA.

Qualifications:

Current RN license in the state of Florida. Minimum 1 year of home health experience with working knowledge of Medicare and OASIS. Fluency in Spanish and English (verbal and written). Strong clinical judgment and ability to work independently in the field. Reliable transportation, valid Florida driver’s license, and auto insurance. Familiarity with EMR systems-Kinnser preferred.

Preferred Qualifications:

BSN degree preferred. Wound care and IV therapy experience a plus. Familiarity with community resources in Indian River and Fort Pierce.
