Admissions RN Position Available In Lake, Florida

Job Description:

The Admissions RN is a registered nurse and an InterdisciplinaryTeam member whose primary responsibility is to present VITASservices and philosophy to patients and families in a manner thatresults in the admission of appropriate patients.

Participation inthe information gathering, analysis, education and form completionaspects of the admissions processEvaluate patients for hospiceappropriateness, e.g. medical records review as requested.

Obtainagreement for admission of referred patients and families who meetadmission criteria.

Analyze data collected by the LPN tofacilitate/obtain admission ordersBuild effective referral sourcerelationships to support the high quality and responsivenessexpectations associated with VITAS.Maintain clinical andprofessional competence.

Take the initiative in developing effectiverelationships internally with the appropriate admissions andpatient care resource persons.

Utilize interpersonal skills to helpappropriately educate and address any questions the patient and/orpatients family has concerning hospice and palliative care.

JOBREQUIREMENTS

Knowledge of the principles of the intake, admission,and transition to care process with the ability to further developskills in these competency areas.

Ability to communicate effectivelywith patients and families, physicians, referral sources andcolleagues.

Knowledge of clinical manifestations of diseases andability to develop VITAS specific clinical knowledge.

Ability to usethe VITAS information system and mobile technologyplatforms.

Reliable means of transportation.

QUALIFICATIONS

Currentlylicensed as a R.N. in the state where duties are performed.

Previoushospital, physician practice, LTC or ALF experienceStrongPresentation SkillsPrevious sales or service experiencepreferredEDUCATIONBachelors degree preferred
