Position Overview The Wellness Nurse plays a pivotal role in supporting the health, wellness, and independence of our independent living residents. This role emphasizes relationship-centered care, proactive wellness strategies, and effective coordination with residents, families, and the care team to achieve the best possible outcomes. Key Responsibilities Conduct comprehensive wellness assessments to promote holistic well-being. Coordinate services and interventions to help residents maintain their independence and enhance quality of life. Lead and participate in collaborative care meetings to promote communication and integrated care planning. Identify residents at risk and implement targeted strategies to mitigate those risks. Serve as a trusted resource and advocate, educating residents and families on health conditions, disease management, and wellness promotion. Develop, coordinate, and deliver wellness-focused programs, including health education and disease prevention initiatives Qualifications Education /

Licensure:

Active RN or LPN license in FL (required)

Experience:

2-3 years of experience in long-term care, post-acute care, or a similar healthcare setting (preferred)
