Key Responsibilities:

Oversee and optimize clinical operations and skilled care planning Lead admissions, assessments, and quality assurance processes Conduct home visits and drive individualized care management Work closely with physicians to manage complex home care needs Mentor and guide nursing staff to deliver top-tier personalized care Partner with the CEO to execute a next-generation home care model Serve as a visible leader in the community through education and outreach Participate in executive-level strategic planning and growth initiatives We’re Looking for a

Qualifications:
