Job Summary:
We are seeking a skilled and compassionate Registered Nurse (RN) to join our team as a Hospital Rounding RN. In this role, you will be responsible for rounding on patients, assessing their needs, and providing high-quality care to ensure excellent patient outcomes. You will work collaboratively with our interdisciplinary team to deliver patient-centered care and promote a culture of safety and excellence.
Key Responsibilities:
- Conduct patient rounds to assess patient needs, provide education, and deliver care
- Monitor patient conditions, identify changes, and intervene promptly to prevent complications
- Collaborate with interdisciplinary teams, including physicians, therapists, and other healthcare professionals
- Develop and implement individualized care plans to meet patient needs
- Provide patient and family education on disease management, treatment plans, and self-care
- Document patient information accurately and efficiently in the electronic medical record
- Participate in quality improvement initiatives to enhance patient care and outcomes
- Maintain a safe and therapeutic environment for patients and staff

Nurse Case Management Requirements:
- Active RN license in the state of practice
- Nursing Experience
- Strong assessment and clinical skills
- Excellent communication and interpersonal skills
- Ability to work collaboratively in a fast-paced environment
- Electronic medical record (EMR) experience
Preferred Qualifications:
- BSN degree
- Experience in a similar role
