Job Description
Case Manager (RN) Leon Medical Centers Miami, FL 33166 The RN Case Manager is a practice-based RN who directly supports Leon Home Health, LLC patients. In collaboration with other members of the healthcare team, the RN Case Manager is responsible for organizing, coordinating, and care management services to patients within the practice.
ESSENTIAL DUTIES AND RESPONSIBILITIES
1.Develops orders that corresponds with the physician’s orders through assessment of patient’s needs, condition, environment, and consultation with other health team members, at time of initial visit and updates plan of care regularly as necessary and documents this care following Agency procedures. 2. Responsible for the delivery of competent, quality patient care in the home. 3. Maintains an ongoing responsibility for assigned caseload. 4. Responsible for the implementation and evaluation of individual patient plan of care. 5. Maintains communication with Nurses, Physical Therapists, Home Health Aides, Staff Coordinator and Director of Nursing, providers, Doctors and mid-level providers (APRN). 6. Schedules work load for maximum efficiency. 7. Participates in Agency evaluation program as instructed. 8. Regularly conducts Case Care Conferences. 9. Abides by and promotes Agency administrative policies. 10. Rotates on -call for a week, including Saturday rotations. 11. Rotates on-call whenever needed. 12. Rotates Holidays as needed. 13. Maintains clinical records, clinical notes and progress reports on each client in accordance with the Agency’s documentation requirements.
QUALIFICATIONS
This position requires self-direction, leadership and the ability to participate as a member of the leadership team. Must possess excellent interpersonal skills to relate effectively with department directors, physicians, administrators and employees. Strong leadership and management skills. Ability to work effectively in a team environment. Excellent oral and written communication skills. Excellent interpersonal skills. Individual must be highly self-motivated and able to work well under pressure.
WORK EXPERIENCE
Minimum of three (1) years’ experience as a licensed, registered nurse.
EDUCATION
Registered Nurse currently licensed in the State of Florida.
LANGUAGE SKILLS
Fluent in English and Spanish.
