Key Responsibilities:

Ensure the highest standard of care for our long-term residents.
Comprehensive Assessment:
Conduct thorough initial interviews and assessments for new residents.
Care Planning:
Develop individualized care plans with both short-term and long-term improvement goals.
Interdisciplinary Collaboration:
Facilitate productive meetings with multidisciplinary teams, caregivers, and families to ensure coordinated care.
Community Engagement:
Work with community specialists to achieve optimal outcomes for our residents.
Leadership and Education:
Lead nursing management team meetings, communicate effectively with all levels of nursing staff, and provide informative in-service education sessions on MDS, RAPS, and Triggers.
Schedule Adherence:
Ensure adherence to the MDS/Care Planning schedule for non-Medicare residents.
Qualifications:
