  Qualifications
  Be the Best Patient Family Liaison You Can Be
  If you meet these qualifications, we want to meet you!
  Meets hospice licensure requirements in the state for any of the following disciplines:
  Social Worker
• Masters Degree required, 1+ yr experience required
  Registered Nurse
• Associates degree, preferred, Registered Nurse license required in state(s) practicing
  LVN
• Associates degree, preferred, Current and valid Licensed Vocational Nurse license in state(s) license(s) where practicing required
  Chaplain
• Bachelor’s Degree from a Council for Higher Education accredited college, university or divinity school is required, Ordained, credentialed, or commissioned according to the practices of a legitimate and recognized religious organization/faith tradition, A minimum of one Clinical Pastoral Education (CPE) unit

Required Certifications and Licensures:

Must be a licensed driver who can travel to all business locations Come As You Are
