The Home Health Registered Nurse provides in-home care for patients, ensuring high quality nursing care while promoting independence and patient education. The RN plays a key role in assessing patients’ needs, developing individualized care plans, coordinating with the healthcare team, and monitoring patient progress. This position requires strong communication skills, clinical judgment, and a compassionate approach to patient care. Minimum Qualifications Bachelor of Science in Nursing (BSN) or Associate Degree in Nursing (ADN) from an accredited program. Current Registered Nurse (RN) license in the state. Minimum of 2 years of clinical nursing experience, with at least 1 year in home health or a related field. Strong clinical assessment and patient care skills. Ability to work independently and manage time effectively. Proficient in electronic health records (EHR) systems. Excellent communication and interpersonal skills. Current CPR certification. Job Responsibilities Conduct initial and ongoing assessments of patient health conditions. Develop individualized care plans in collaboration with patients, families, and healthcare providers. Administer medications and treatments according to physician orders and best practices. Educate patients and families on disease management, medications, and care techniques. Monitor and document patient progress and adjust care plans as needed. Coordinate with physicians, therapists, and other healthcare providers for comprehensive patient care. Supervise and provide guidance to Home Health Aides and LPNs. Ensure compliance with home health regulations and company policies. Assist with discharge planning and provide follow-up care coordination. Maintain accurate and timely documentation in electronic health records. Ensure compliance with the organization’s Code of Conduct and all applicable laws, including HIPAA regulations for the protection of sensitive information. Perform any additional duties as assigned to meet the needs of the department and organization.
