Contract Position Overview:
We are seeking a skilled Case Manager RN (Triage Nurse) who will play a pivotal role in supporting our mission to reduce overall admissions, readmissions, and ER visits. This position is vital in managing acute calls and providing clinical support for acute visits onsite, with a focus on delivering exceptional patient care in a triage role.
Key Responsibilities:
As the Clinic Nurse (Triage Nurse), you will: Provide clinical triage support in both clinic and telephonic settings. Maintain an active nursing appointment schedule to assist Providers in monitoring patients’ healthcare needs. Collaborate with Care Managers and Providers to ensure comprehensive follow-up for post-hospitalization or post-emergency room visits. Conduct comprehensive screenings for care opportunities, working with Physicians and RNs to develop tailored care plans. Deliver condition-specific patient education, anticipatory guidance, and care coordination under the direction of Providers and RNs. Ensure accurate and compliant clinical documentation within patient medical records. Provide health education to patients. Identify care management referral opportunities in collaboration with Physicians.
Required Skills and Attributes:
Active FL RN License Strong verbal and interpersonal communication skills to interact professionally with patients and staff at all levels. Ability to work independently in a fast-paced environment, demonstrating strong organizational and time-management skills. Proficiency in MS Office and Excel. Knowledge of medical terminology and HIPAA regulations. Familiarity with medical equipment and willingness to learn equipment maintenance. Ability to complete a color blindness test successfully.
