Key Responsibilities:
Elevate Care Standards:
Oversee the care delivery to ensure our residents receive top-tier support tailored to their individual needs.
Conduct Thorough Assessments:
Complete detailed assessments and interviews for new residents, crafting customized care plans that set the stage for success.
Design Impactful Care Plans:
Develop individualized care plans with measurable goals, focusing on both short-term and long-term resident improvement.
Collaborate Across Disciplines:
Lead interdisciplinary meetings with healthcare professionals, families, and caregivers to develop holistic, effective care strategies.
Drive Resident Outcomes:
Work closely with specialists to ensure the best possible care results for our residents.
Provide Leadership & Education:
Lead nursing management meetings and facilitate in-service training on MDS, RAPS, and Triggers, empowering the team with knowledge and skills to stay ahead.
Ensure Compliance:
Maintain the MDS and care planning schedules for non-Medicare residents, ensuring consistency and regulatory compliance.
What We’re Looking For:
Licensed Registered Nurse (RN) in Florida A minimum of 2 years of experience in a skilled nursing facility (SNF) Solid understanding of MDS, Medicare, and the care planning process
