- Surgical Care Coordinator
JOB SUMMARY
The registered nurse (RN) provides care coordination for the surgical episode of care for HSS Florida Physicians, LLC patients.
Primary responsibility includes:
Needs Assessment for Patients Scheduled for a Procedure or a
Low Risk Surgery:
provides ad hoc assessment for care coordination needs at the request of the provider and creates individualized care plan Needs Assessment for Patients Scheduled for a
Moderate or High-Risk Surgery:
assesses every patient for health complexity and surgical risk to determine appropriate care pathway for the patient and tailors the pathway to meet the patient’s individual needs Surgical Care Coordination (Standard) Ensures patient has appropriate and timely pre-op consults and testing scheduled Timely review of results for all pre-op consults and testing and either recommends patient for: final sign-off by facility anesthesia team additional consults/testing Collaborates with outside providers, surgeon, and facility team to ensure one comprehensive list of pre-op instructions for the patient Directly provides patient pre-op instructions and/or collaborates with facility team to ensure completion Serves as patients’ primary point of contact for clinical questions both before and after surgery Surgical Care Coordination (Advanced) All components of standard care coordination Pre-op phone assessment and care planning for advanced surgical needs (equipment, medications, therapies, home services, etc) Assists in set-up and coordination of the additional services Post-op calls to assess patient status and provide additional post-op education Leads Care Coordination Meetings Organizes and leads weekly meetings with surgical facilities to review patient status and coordinate care needs Participates in Development & Maintenance of the Surgical Care Coordination Program Assists in creation and maintenance of care algorithms, care pathways, and patient education materials Identifies opportunities for improvement and assist in developing and implementing potential solutions Visits and liaisons with local outside providers to improve pre-op processes Weekend and Holiday Patient Calls Post-op discharge calls are based on the timing of surgery and therefore do also fall on weekends and holidays. A schedule for weekend and holiday rotation will be maintained within the department. As-needed Coverage for administrative support in the absence of the Medical Secretary Other duties as assigned
EDUCATION REQUIRED BSN. CERTIFICATIONS/LICENSURE REQUIRED
Active Florida RN license and BLS from
AHA. YEARS OF EXPERIENCE REQUIRED 1
year of RN experience required.

EDUCATION PREFERRED CERTIFICATIONS/LICENSURE PREFERRED SKILLS REQUIRED
Ambulatory orthopedic and pre-surgery optimization clinical competency, critical thinking, patient education regardingplan of care and clinical pathway, attention to detail, selfmotivated and proactive.
