As an integral part of our team, the RN Case Manager will play a crucial role in providing exceptional care to our patients. This position involves a dynamic blend of office and field responsibilities, offering a unique opportunity to engage with both our healthcare professionals and patients.

Responsibilities:

Patient Care:

Conduct initial evaluation visits and reassess patients’ nursing needs regularly. Coordinate and collaborate with healthcare professionals to develop and implement individualized care plans. Monitor patient progress and adjust care plans as needed. Provide hands-on patient care when necessary.

Documentation:

Prepare accurate and timely clinical and progress notes for each patient visit. Summarize care conferences promptly following agency policies and procedures. Maintain comprehensive and up-to-date patient records.

Communication:

Collaborate with the Director of Clinical Services/Nursing Supervisor to ensure effective communication and coordination of care. Communicate with patients, families, and healthcare team members to provide updates and address concerns.

Field and Office Time Management:

Effectively balance time spent in the office and the field. Attend regular team meetings and training sessions. Ensure compliance with agency policies and standards.

Qualifications:

Be fluent in English and Spanish. Active and unrestricted RN license in the state of Florida. Knowledge of Pediatric care. Comfortable with using a computer. Strong communication and interpersonal skills. Knowledge of home healthcare regulations and standards. Willingness to be on call as needed. #
